How to get your medical records from Cleveland Clinic

Option 1: MyChart (fastest, free)

In MyChart, the path is Menu, then Document Center, then Release of Information Authorization Form. It works from a web browser only; the mobile app does not carry it. The portal's Sharing Hub also offers a quick snapshot, but a snapshot is not the complete file. The full chart, what HIPAA calls the designated record set, comes through the formal release request.

No MyChart account? Cleveland Clinic also takes an electronic authorization through a web form for non-portal users.

Option 2: The Authorization for Release of Medical Information form

The paper path is the Authorization for Release of Medical Information form. Spanish and records-from-other-facilities versions sit on the same medical-records page. One warning: an outdated 2015 copy of this form still ranks in search results. Use the current PDF linked here.

Option 3: Phone

The records line is 216.444.5580, Monday through Friday, 8:00 to 4:30, with an after-hours line at 216.444.5582. For a full export of your electronic health information, the EHI export desk answers at 216.445.9782.

Fees

From Cleveland Clinic's medical-records procedures: release through MyChart is free. Emailed copies cost a flat $6.50 plus tax. Mailed patient copies are billed per page: $0.36 for pages 1-50, $0.26 for pages 51-200, and $0.12 past 200. Third-party requests carry an $18.91 search fee plus their own per-page tiers.

The state backdrop is Ohio Revised Code 3701.741, which sets the tiers and caps digital-record access at $50. Ohio adjusts these amounts every year, so expect the exact numbers to move. Federally, HIPAA permits only a reasonable, cost-based fee for patient copies, and treats a flat $6.50 as an acceptable charge for electronic ones.

How long it takes

Electronic requests take up to 3 business days. Paper requests take up to 10 days, with 7 to 10 business days typical.

The federal backstop is the HIPAA right of access, 45 CFR 164.524: action within 30 days, one 30-day extension with written notice. If day 31 passes with silence, file a complaint with the HHS Office for Civil Rights.

Getting a family member's records

The entry point is “Request Family Access” in MyChart Messaging, or the front desk at a child's pediatrician. It covers minors through 17, dependent adults, and adult caregivers.

• Teens 13 to 17: parents keep access, but clinical notes are withheld from the proxy view.
• At 18, parental access is removed automatically. The adult patient re-grants it with a signed adult proxy form.
• Dependent adults: a paper caregiver form plus court or power-of-attorney documents, sent to Health Data Services. Allow 1 to 2 weeks.

Which Cleveland Clinic?

The name covers hospitals on two continents, and the records desks are not unified:

• Imaging is its own path everywhere: order online at MyImages or call 216.444.6651, and allow 48 hours. Since April 14, 2026, radiology results inside MyChart open in the Agfa Xero viewer.
• Florida runs per hospital: Weston (roiwestonfl@ccf.org, fax 954.689.5519), Indian River (irrecordrelease@ccf.org, 772.789.8479), Martin Health (772.223.5945 x13070). At Indian River, MyChart only holds records from September 14, 2020 forward; older charts need the paper request.
